Patient form – patient partner match

Patient partners in research are a group of people with personal experience of a health issue (including informal caregivers) who work with a team of researchers or decision makers to ensure that the best interests of patients remain at the centre of their work. Patient partners are valued members of the team who offer a unique perspective and should not be confused with research participants, otherwise known as study subjects.

If you would like to contribute to health research as a patient partner, either by contributing to a research project or by contributing to the development of research priorities, please provide the following information (this information is only used by the Francophone Initiative to match you with a team and will not be shared without your consent)
